OKLAHOMA CITY, Okla., April 12, 2016 (SEND2PRESS NEWSWIRE) -- American Fidelity recently learned of an incident that involves personal information of some of our Customers. The information did not include Social Security numbers.
On February 15, 2016, debit card substantiation letters were mailed to some of our Customers. An error in the mailing process caused some of our Customers to receive information that was intended for a different Customer. This means that a portion of one Customer's letter may have been inadvertently mailed to another Customer. While it is believed misuse of this information is unlikely, American Fidelity is sharing this information out of an abundance of caution.
The letters included information about Customers' recent flexible spending account debit card usage. The information may have included their name, address, employer name, employer ID number, the last four digits of their debit card number, a date of service, a provider name and a payment amount. Customers who received another Customer's information have been asked to contact American Fidelity so that arrangements can be made to collect it.
American Fidelity is offering affected Customers a year of credit monitoring and identity protection services. Affected Customers were mailed details about this disclosure along with instructions on obtaining assistance with their free credit monitoring and identity protection services.
American Fidelity has concluded that this incident was a result of human error, and has taken steps to prevent this from happening in the future.
