How Intermountain Healthcare Is Making Care More Affordable

SALT LAKE CITY, Utah /ScoopCloud/ -- Making healthcare more affordable for the people it serves is a top priority for Intermountain Healthcare. That's what Intermountain CEO Marc Harrison, MD, said at meetings held this summer with managers from across the Salt Lake City-based health system.

Harrison told managers that Intermountain is well-positioned to lead the nation in providing the best care at an affordable cost, and, with the help of each caregiver, it's a goal that can be achieved.

"We have a crisis in American healthcare right now," Dr. Harrison said. "Healthcare is unaffordable. The existing healthcare system is oriented toward driving volume. As an industry we do too many things to too many people that they really don't need, which hurts them economically, and sometimes physically. But Intermountain has a great history of innovation and together we can drive change."

Dr. Harrison shared some alarming national statistics about the cost of healthcare:
* One in four people will forego medical care this year because it's too expensive.
* One in three Americans ration their prescription drugs because they can't afford them.
* About half of Americans worry an unexpected healthcare expense will bankrupt them.
* Healthcare expenses are the number one cause of bankruptcy in the United States.

Dr. Harrison says the statistics are better in Utah and Intermountain's charity care program is helping those who can't afford care, but he says more needs to be done. Last year, Intermountain provided more than $246 million in charity care to patients in need of financial assistance.

"We're doing a great job with the cost of healthcare and Intermountain is as competitive and cost-effective as any large system in the United States," he said. "But we have a long way to go and we're going to drive this forward together. How are we going to get there? By keeping people well, by providing the most accessible and affordable care, and by creating a sustainable model."

Examples of innovative solutions at Intermountain that are reducing costs and improving care, which Dr. Harrison shared with managers:

* Civica Rx, the not-for-profit generic drug company banded together by Intermountain and other healthcare systems and philanthropists to fight the skyrocketing cost of prescription drugs by providing low-cost generic drugs to participating hospitals.
* Reimagined Primary Care, the team-based care model that focuses on preventive care to keep patients well. The model already has about 25,000 patient members enrolled and has seen impressive results, such as a 60 percent decrease in hospital admissions, 35 percent decrease in emergency department admissions, and a 20 percent decrease in per member per month costs.
* Addressing social determinants of health such as stable housing, food, transportation, and employment.
* Reducing opioid misuse. Intermountain has reduced the number of opioid tablets prescribed for acute care by more than 30 percent in the last year and is working hard to make further reductions.
* Integrating mental health care into clinics as part of each patient's primary care experience.
* Finding and implementing caregiver ideas. Intermountain caregivers implemented nearly 49,000 continuous improvement ideas in 2018 - more than 130 ideas each day. Many of these ideas both improved quality and reduced expenses.

"We didn't do these things for our self-aggrandizement," Dr. Harrison said. "We did them for our neighbors. And we're in just the right position to continue to be an innovator."

Dr. Harrison added that although helping people stay well and out of the hospital is a key part of Intermountain's strategy moving forward, that doesn't mean we won't continue to provide the best possible hospital care when our patients need it.

"We'll still focus on helping people when they're in a car accident, have a heart attack, or get cancer," he added. "But we have the opportunity to do that in conjunction with a 'keep people well' system. We're still going to have great trauma programs, great heart surgery programs, great cancer care programs, and great neuro ICUs, but won't it be terrific if we can actually keep people out of some of those places? And have them stay as close to home as possible in the least restrictive environment?"

Dr. Harrison emphasized that Intermountain is making changes from a position of strength.

"Intermountain, in a really tough environment, is really doing well. Our quality and safety have never been better. Our patient experience is on the rise. At a time when a lot of other health systems are struggling economically, we're strong. But we need to continually innovate and change to remain a model healthcare system. If not us, who? If not now, when?"

Other things Intermountain and SelectHealth (Intermountain's insurance arm) are doing to reduce costs and return money to its patients and members:

• Lower annual rate increases. Intermountain reduced annual price increases in 2019 by $15 million.
• Lower-cost care settings. Intermountain is reducing unnecessary ER visits by increasing access to appropriate care in lower-cost settings such as InstaCares (urgent care clinics), physician offices, and Connect Care virtual visits.
• Lower individual premiums. SelectHealth reduced individual plan premiums by an average of 2.7 percent in 2019.
• Affordable SelectHealth Share premiums. Intermountain guarantees a premium rate increase for new clients of SelectHealth Share of only 2.5 percent in years two and three of their three-year contract. For current clients, the guaranteed premium rate increase is only 2 percent for all three years. This compares to an industry-wide premium increase of more than 5 percent.
• Stork Savers. Intermountain's Stork Savers program offers a lower-cost cash price to patients without health insurance for mom and baby care associated with uncomplicated births.
• Supply initiatives. Intermountain has a set ratio between what it pays and what it charges patients for supplies and drugs. When Intermountain reduces its costs, it can directly lower charges to patients for those items. Intermountain negotiated better prices on supplies in 2018 that resulted in more than $32 million in cost savings.
• Financial assistance. Intermountain expanded its financial assistance program to cover needed health services for families with income levels up to 200 percent of the national poverty level. The 2019 national poverty level for a family of four is $25,750.

Ohio Marijuana Card to Give Away 15 Free Evaluations Per Month to Those in Need

Owen Anketell:
Owen Anketell trained at UHP, and during the summer of 2017 hand cycled 2,365 miles from Calais, Maine to Key West, Florida to promote awareness for his own organization - Adaptive Sports Awareness. Owen was born with hereditary spastic paraplegia, so weak leg muscles necessitates using a wheelchair, but this has not stopped him from succeeding as an athlete.

Owen continues to be actively involved in sports. He attends the University of Arizona which has one of the best adaptive sports programs in the country. Owen, along with his best friend and cousin, averaged 50 miles a day during their ride to Florida.

Nick Giovinazzo:
Nick Giovinazzo, Best Buddies and Special Olympics Massachusetts participant, is another remarkable adaptive athlete at UHP who has attained impressive feats. Nick can deadlift more than 300 lbs., bench press 185 lbs. and can back squat nearly twice his bodyweight.

More than an impressive athlete, Nick is an incredible advocate of inclusion; participating in the Best Buddies Football Challenge and starring in their commercials alongside New England Patriots Superstar Tom Brady.

San Antonio Podiatrist Washes Away Tough Cases of Heel Pain with Water

Arizona Surgeon Invents and Performs World Record Setting Cancer Surgery

MESA, Ariz. /ScoopCloud/ -- The World Record Academy recently awarded Dr. Greg J. Marchand and his surgical team a world record for removing a 17-centimeter cancerous ovarian tumor using only small incisions in the abdomen, a technique called a laparoscopy. While removing cysts and tumors using laparoscopic surgery is a standard practice, Dr. Marchand said it is rare to use the technique in conjunction with an ovarian cancer staging procedure.

The surgeons completed the procedure using "in-bag" morcellation, a technique developed by Dr. Marchand in which the tumor is bagged and broken down inside the bag so the pieces can be safely removed through tiny holes. Otherwise, patients would need a sizeable incision to surgically remove such a large mass, requiring additional recovery time and a great deal more postoperative pain.

Marchand, a board-certified OB-GYN specializing in "minimally invasive surgery," worked closely with a gynecologic oncologist in completing this amazing surgery.

Marchand said the most difficult part of the surgery was removing the cancerous mass without spilling any cancer cells into the abdomen. If cancerous material spilled back into the abdomen the cancer could have spread and worsened the patient's prognosis.

The World Record Academy helped verify that this type technique had never been accomplished previously on any malignant tumor this large.

Dr. Marchand was diagnosed in 2010 with mixed-cell carcinoma, (an aggressive cancer of the testes,) said it is particularly gratifying that his technique can help cancer patients. He received surgery and treatments in 2010, and has been in remission since.

"I know what it's like to face a diagnosis of cancer," he said. "In my opinion, advancements in the surgical treatment of cancer are just as important as the newest cancer-fighting drugs and chemotherapy agents."

"If we can use minimally invasive surgery to take some of the recovery time and complications out of cancer surgery, then I think we've really done a lot of good for patients fighting cancer."

This is Dr. Marchand's second world record in laparoscopic surgery. In 2008 Dr. Marchand was awarded a Guinness World Record for the removal of the largest uterus laparoscopically. In this case, it was not a cancerous mass, but a seven-pound benign (non-cancerous) uterus that was removed.

Why did it take so long for the World Record to be awarded?

To verify this record, we had to research all the other related surgeries that had been published to determine if ours was truly the largest malignant tumor ever surgically staged through completely laparoscopic means. After our research determined it was, we employed the World Record Academy who underwent their own verification process. Following the completion of these two independent verifications, we can now confidently say that this is likely the largest malignant ovarian tumor to ever undergo completely laparoscopic surgical staging.

What is "In-Bag" morcellation, how is it different from just breaking something apart, or regular morcellation?

In-Bag morcellation refers to the surgical technique of placing an object inside a bag, usually plastic, before breaking it into pieces. Usually the object is broken into small pieces so that it can be removed through small holes, thus not needing to make a large incision to remove the object. This high level of caution is reserved for objects that could contain cancerous cells. In the case of suspicion of cancerous cells, it is important not to spill the material back into the patient's body cavities because cancer cells can "Seed" or begin new cancer colonies if they are moved around the body. In this case, we removed a 17-centimeter cancerous tumor through a hole that was approximately 1.4cm large. This technique is one of the more extreme examples of "In-Bag" morcellation.

What does this mean to you as a cancer survivor?

"I underwent surgery and treatments after I was diagnosed with Mixed Cell Testicular Cancer in 2010. I am very lucky that the surgery for my stage of Testicular cancer was a very minimally invasive procedure, and that I had a good surgeon. I remember the horrible fear of 'just not knowing what was going to happen,' and I think that fear is really worse than any of the pain of the cancer or the treatments. When I woke up from the surgery, I really felt great, and over the next few days I had a very quick recovery.

"I was able to get back into my life quickly, and although I was still afraid of what was to come, I felt like I had the strength to fight. When I think about my patients with Ovarian cancer, I would like them to be able to come out of surgery feeling like they have the strength to fight, to take on the cancer. I can imagine that when you are recovering from a one of these big, open traditional staging procedures, you're in excruciating pain, you really can't move, and you like you've just been hit by a bus. I'm sure you don't feel strong and ready to fight the cancer at that point. You might just feel like giving up. If my technique can help even a few patients avoid that feeling, then thats the most valuable thing in the world to me.

"That's why I'm really passionate about what this new cutting edge technique can do for cancer patients, and I feel it can be just as valuable in the fight against cancer as the latest 'big pharma' wonder drug."

Missing Pieces of the ‘Health Care’ Debate

Dr. Harte explains, "Big Government subsidizes Big Pharma's research, from which they make huge profits, which translates into huge profits for the medical industry as they use those pharmaceuticals, with prices so high that government needs to subsidize both Medicine and Big Pharma. All the while, government makes believe that they are regulating these industries, while they offer them protection from liability, and make some of their services, like vaccination, compulsory. Mass media now lives off the drug ads, as they breathlessly gush about new medical 'breakthroughs.' Together, they form what I call the PMMG, the Pharmaceutical-Medical-Media-Governmental Complex. How about we drain that swamp?"

"As the current 'health care' discussion rages on," Dr. Harte observes, "there is a great deal of emphasis on 'keeping costs down... affordability.' While the Affordable Health Care Act has proven to be not affordable, neither will what is being proposed now." He declares "Modern Medicine, by its very nature, cannot be affordable because it continually expands the demand for its services, by causing more sickness. And what kind of industry makes itself unaffordable?"

"The screaming truth that neither political party faces is that Modern Medicine is an abysmal failure across the board. Amongst the old," Dr. Harte states, "there is the terrible epidemic of Alzheimer's, which, in my opinion, is caused by a combination of commonly prescribed drugs that force blood pressure too low, the effects of the cholesterol-lowering drugs, too many psychiatric drugs, and too many exposures to general anesthesia for mostly unwarranted surgical procedures. Look at the current horrible health of children, from autism to Type I diabetes to life-threatening allergies. Trace that to the serial poisoning of children by the known neuro- and immunotoxins in the dozens ofvaccines they are forced to take. The rest of us? We seem to see the doctor, and take more long-term drugs, than any previous generation, yet we are sicker than ever, mentally as well as physically! Nixon declared a War on Cancer in 1972. How's that working out?"

"There is a demand for more 'access' to 'health care,'" says Dr. Harte. "Perhaps we would be better off... healthier... with less access. 'Pre-existing conditions?' A lot of these pre-existing conditions are caused by medical care, and a lot could be at least partially resolved outside of medical care, particularly with chiropractic care, which allows the body to heal itself by liberating its master system, the nervous system."

"The assumption," Dr. Harte states, "is that Medicine is health care. The biological reality is that Medicine is sick care, not health care." Dr. Harte goes on, "the crux of the confusion is that we've been lead to believe that health is the absence of disease, or as an even more limited conception, the absence of serious disease, or of symptoms. No, it is not!"

Dr. Harte declares, "The solution to the so-called health care crisis is to promote actual health, through regular wellness chiropractic care, a more natural, non-GMO food supply, etc., while ending the immoral and extralegal government protection, promotion and support of Medicine and Big Pharma.
