Podiatric Group Brings Modern Approach, Old Charm to Horizon West with New Clinic

One such service is the miniBunion™, a minimally-invasive procedure for bunion correction. The practice also offers restorative medicine and on-site diagnostic equipment, including digital X-ray and ultrasound technologies.

"Since January, we've expanded from one office in Tampa Bay into a multi-regional group," said Dr. Siegel. "We're able to grow like this due to our reputation of prioritizing compassionate, personalized attention and supporting that care through innovation."

Dr. Patel said her group's modus operandi was inspired, in part, after noticing more healthcare corporations replacing provider-run practices in recent years.

The difference between Modern Foot & Ankle and corporate-owned clinics, Dr. Patel explained, is that "our podiatrists have full control over how we deliver care and aren't governed by quota requirements or the pressure to upsell." She added that corporations are known for micromanaging their healthcare providers.

"We're going through a lot of change right now, but my partners and I will never compromise on how we treat those who come to us for help," said Dr. Patel. "My duty is to the patient's well-being, not the bottom-line."

She also noted that the staff is vaccinated against COVID-19, and each office will continue to follow all CDC guidelines to help protect patients from the virus.

Dr. Patel's former clinic in Casselberry will remain a Modern Foot & Ankle location. Dr. Jacob Huffman has taken over as the office's sole practitioner.

Individual with Down Syndrome Testifies for Phasing Out Subminimum Wages for Individuals with Disabilities

WASHINGTON, D.C. /ScoopCloud/ -- The National Down Syndrome Society (NDSS), the leading organization for all individuals with Down syndrome, and the Massachusetts Down Syndrome Congress (MDSC), the premier resource for information, advocacy and networking in Massachusetts, applaud the testimony of John Anton at Wednesday's Education & Labor Committee joint subcommittee hearing titled "Phasing Out Subminimum Wages: Supporting the Transition to Competitive Integrated Employment for Workers with Disabilities."

Mr. Anton, who works as MDSC's legislative specialist, has been a vocal advocate on issues concerning the rights of individuals with Down syndrome. His testimony is an important part of efforts by the community to support the passage of the Transformation to Competitive Integrated Employment Act (H.R. 2373) which was introduced by Congressman Bobby Scott (D-VA) and Congresswoman Cathy McMorris Rodgers (R-WA).

At the hearing, Mr. Anton spoke about the vast difference between his work at MDSC and his experience working in a sheltered workshop. While working for subminimum wages, he was bored and asked his boss for more work. He was told no and quit his job on the spot. He went on to work for the Massachusetts State House and intern in Congresswoman McMorris Rodgers' office prior to his role at MDSC.

"It is all about respect and dignity as a citizen for me and not being stigmatized by labels, which belong on jars not people!" said Mr. Anton.

"We are thrilled that John had an opportunity to lend his powerful voice in this important forum," said MDSC Executive Director Maureen Gallagher. "Being paid an unfair wage is, unfortunately, a topic that John knows all too well. But not only has he survived, he has become a leading spokesperson in the movement to ensure that all people with Down syndrome and other disabilities have equal access to employment and other opportunities." Gallagher added that MDSC is also working on programmatic solutions to the disability employment crisis. "Through our Your Next Star employment initiative, MDSC is outreaching directly to employers to open their eyes to the abilities of employees with Down syndrome and help them diversify their workforces," she said.

Section 14(c) of the Fair Labor Standards Act (FLSA) of 1938 allows employers to pay people with disabilities less than the minimum wage (subminimum wage). People with disabilities who are employed under the 14(c) certificates are trained to perform mundane tasks for as little as $0.02 an hour, according to a 2018 report from the National Council on Disability.

"Despite being referred to as training programs, these environments do very little to build capacity or transfer into skills necessary to help people transition into other employment options," said Matteo Lieb, Employment Policy and Program Manager at NDSS. "This practice reinforces the misconception that individuals with disabilities are less productive and creates an artificial barrier to future employment opportunities."

As support builds for wage equity for people with disabilities around the country, NDSS, in partnership with the MDSC and other grassroots advocates, will continue to support phasing out subminimum wage at the federal and state levels.

"Equitable employment is a key legislative priority for NDSS," said President & CEO Kandi Pickard. "We believe every individual deserves the opportunity to work in an inclusive, competitive role where they can acquire meaningful skills and are paid a fair wage. Phasing out 14(c) will help pave the way for equality in the workplace for people with Down syndrome and other disabilities."

Importantly, the study is replicative in nature, after earlier research demonstrated that hydrogen water and caffeine were equivalent overall in raising attention in sleep-deprived individuals yet had an impact on different domains of alertness: Hydrogen improves orienting to sensory stimulation, while caffeine alters awareness and executive attention that refers to the ability to regulate responses in conflict situations.

The randomized, placebo-controlled cross-over pilot trial compared the acute effects of single-dose HRW, caffeine, or HRW and caffeine combined against a placebo beverage on parameters of attention and brain metabolism following sleep deprivation. HRW displayed a more robust effect on brain metabolism as compared to caffeine, leading to higher choline-to-creatine ratio levels in the frontal and paracentral brain as measured via MRI brain scans.

Additionally, HRW was superior to caffeine and the placebo in reducing the number of errors in the symbol digit modalities test (SDMT) and trail making test in the sleep-deprived individuals. HRW displayed no side effects and, therefore, might be advanced as a safe and effective alternative to caffeine to address some of the effects of sleep deprivation, with more studies on the subject highly warranted.

The trail making test is a neuropsychological test of visual attention and task switching, while the SDMT detects cognitive impairment. Brain metabolism was evaluated with proton magnetic resonance spectroscopy, a non-invasive gold-standard method to assess changes in creatine, choline, and N-acetylasparate across the brain.

Sergej Ostojic, M.D., PH.D., Professor of Medicine at the University of Novi Sad, a senior author of the Food Science & Nutrition study said, "This was another study by our group that confirmed possible nootropic effects of hydrogen-rich water in sleep-deprived men and women; these effects appear to be accompanied by notable changes in brain metabolism, with frontal brain specifically targeted by HRW. Dissecting Inflammation to Understand Chronic Disease in Type 2 Diabetes

LA JOLLA, Calif. /ScoopCloud/ -- Scientists are pursuing novel strategy to understand the source of inflammation in chronic diseases at the molecular level. Dr. Marcelo Freire at The J. Craig Venter Institute, is among the scientists conducting this provocative research and he's the lead and senior author of a recent study, "Transcriptomics of type 2 diabetic and healthy human neutrophils."

Type 2 diabetes (T2D) is a chronic inflammatory disease affecting approximately 415 million people worldwide, and its prevalence is rising in adults, increasing the risk of developing co-morbidities such as inflammatory lesions on limbs, eyes and periodontal tissues. In fact, patients with diabetes are more prone to develop severe forms of COVID-19. Recently neutrophils have also been associated with inflammatory pathways in Sars-CoV-2 infections.

Thus, demonstrating that diabetics fail to respond to microbial/viral and metabolic challenges due to deficiencies on immune cells. However, clinicians are unable to accurately predict which patients will develop chronic inflammatory and associated co-morbidities.

"Unravelling immune and lipid mechanisms of diabetes can advance our understanding of the disease," Freire says. "We found that aberrant inflammation is a molecular source of chronic disease in type 2 diabetes and that's reversible."

In laboratory studies, researchers identified two pathways that dysregulate inflammation in type 2 diabetes. By employing a global analysis of RNA (transcriptomics) from innate immune cells, they discovered novel immune and lipid-related genes that are chronically suppressed.

Though chronic diseases such as type 2 diabetes have excellent therapeutic modalities that focus on blood sugar levels and insulin, none currently control the disease-associated chronic inflammation.

In this study, human neutrophils (the most abundant leukocytes in the blood) were evaluated via transcriptomics for the first time. This new survey of human neutrophils provides access to the scientific community about regulatory signatures and mechanistic information of in health versus disease.

"Collaborative efforts have been established to expand the knowledge in our lab and others to expand the field of neutrophils and investigate the source of inflammation to other organs," Freire says.

Inflammatory networks derived from neutrophils can become informative in terms of early diagnostics of diabetics and future development of therapeutics.

For this work, the researchers treated human cells ex vivo with a novel fatty acid, resolvin E1 (RvE1), known for its pro-resolution functions. The application of this exogenous therapeutic showed that type 2 diabetics are not irreversibly damaged and that the treatments that reduce chronic inflammation may rescue the phenotype. Because Freire's team has expertise in inflammation and the receptor-ligand axis, they were able to assay human cells with tailored amount of RvE1 to further advance precision medicine.

In addition to conducting RNA sequencing (transcriptomics) and analysis of full neutrophil transcriptomes, the team used cell culture to enhance concentrations of RvE1 for both healthy and diabetic neutrophils The scientists tested control treatments to account for placebo stimulation of cells, but genes that were deficient or saturated in diabetic subjects only changed after specific concentrations of RvE1 were added, demonstrating that chronically inflamed cells are responsive

"There's much more research ahead, but the findings around unresolved inflammation should be a starting point to develop novel biomarkers for chronic disease severity and treatment," Dr. Sarah Kleinstein, first author of the study, says.

Freire agrees that the research is just in the beginning and future plans include furthering their understanding of diabetic heterogeneity and diagnostic/therapeutic development.

"We aim to find a better solution to monitor and treat chronic inflammation," Freire says.

Dr. Haiavy is only the third surgeon to ever place silicone implants through the discreet incision in the navel.

It was previously believed that only unfilled saline breast implants could be placed through the small incision in the umbilicus, with the saline filling being added through a tube after placement. Silicone gel-filled implants, on the other hand, are pre-filled, and were thus thought to require a larger incision than is possible through the transumbilical plane. This left patients desiring the natural, lightweight feel of silicone breast implants with prominent incisions on or near the breasts.

The addition of the transumbilical incision option for silicone gel-filled implants is a significant milestone in the history of breast augmentation; since attaining FDA approval in 2006, silicone implants have become the preferred implant of hundreds of thousands of breast augmentation patients every year. Not only is the small TUBA belly button incision inconspicuous compared with incisions around the breasts, but Dr. Haiavy believes this technique also facilitates an easier recovery compared with traditional augmentation methods.

"A 'scarless' transumbilical approach appeals to so many patients, but many were torn between this preferred incision option and their desire for the wonderful feel of silicone implants. I am delighted to report patients can now have the best of both worlds," explains Dr. Haiavy.

Transumbilical silicone breast augmentation (TUSBA) is performed along the body's natural planes.

After over 20 years of experience with TUBA using saline implants, Dr. Haiavy collaborated with colleague Gabriel H. Patino, MD to develop the technique for placing silicone implants. He has dubbed the technique for placing silicone implants through a transumbilical incision "TUSBA," which stands for "Transumbilical Silicone Breast Augmentation."

Like TUBA, the TUSBA surgery is performed using blunt instruments to create a tunnel from the incision in the belly button to the breast. Through this tunnel, a pocket is created under the chest muscle, the breast pocket is expanded, and then the work is carefully inspected using an endoscopic camera.

Finally, the silicone gel implants are placed using the Keller Funnel(r) No-Touch(tm) technique, in which the implant is transferred directly from its sterile packaging using a hydrated plastic funnel, which is much like a piping bag used for frosting a cake. The Keller Funnel(r) allows the implant to be placed gently, with excellent precision and zero direct handling by the surgeon, thereby reducing the risk of infection.

While patients will experience a similar soreness in the breasts as they do after other breast augmentation techniques, there is minimal discomfort in the abdomen, Dr. Haiavy explains. He prescribes routine recovery measures, but finds that patients have an easier recovery overall after TUBA versus other augmentation methods.

There is one caveat to the technique: Dr. Haiavy limits TUSBA patients to a maximum breast implant volume of about 400 cc, or an increase of roughly two cup sizes. Still, he says, this is a substantial enough increase that many patients will be able to reach their desired outcome with TUSBA.

"Good candidates for TUSBA breast augmentation include non-smokers in good health who want the natural feel of silicone breast implants without visible scarring. It would also be an excellent choice for those who want to replace lost breast volume after pregnancy or weight loss. I am confident I can achieve an excellent outcome for patients requiring silicone implants of 400 cc or less."

Chicago Support Group Part of National Network Taking Aim at Chronic Pain

The support groups offer attendees the chance to connect and talk with others who understand the unique challenges of disabling pain. But there's also an educational component-leaders help participants understand new treatment options, self-management strategies, mindfulness techniques, and more.

"I have lived with chronic pain for 17 years. For many years, I felt alone and feared my chronic pain would make my life unmanageable. I felt like I lost so much of my life. It's been a long road to get to the place I'm in now, and I hope to help others on their path towards a better life while living with chronic pain," says Fleming.

The group is part of Pain Connection, a national network of support groups created through U.S. Pain Foundation, the largest organization in the country dedicated to serving people with chronic pain. Led by Gwenn Herman, LCSW, DCSW, who lives with chronic pain herself, Pain Connection offers an intensive training to volunteers looking to become effective group leaders.

After completing the training program, support group leaders return to their communities ready to establish a group for other people with chronic pain. The leaders use the training and their own experiences with chronic health conditions to facilitate dialogue and understanding among group members, and nurture a sense of community that can be so valuable.

Timothy currently serves as an Advisor to the UT Dell Medical School Catalyst program, the ACC Bioscience Incubator, and is a founding member of the TEXGHS Global Health Security Consortium. Further expansion of the BioAustinCTX board is likely in the coming year.

Mouth bacteria could make stronger vaccines

HAMPTON, N.J. /ScoopCloud/ -- Three researchers have published a groundbreaking paper https://www.nature.com/articles/s41541-021-00341-4 on the importance of oral bacteria in the fight against infectious diseases like influenza, HIV, and COVID-19, Gingivax LLC announced today.

The bacteria in our mouths have traditionally been recognized as a nuisance or worse. They are the cause of tooth decay, gingivitis, and periodontitis. Since we were kids, we were taught to vigilantly brush our teeth twice a day to keep them healthy.

But it turns out that oral bacteria also contain a secret: Research indicates that they possess the tools necessary to boost our immune systems against nastier pathogens, like the coronavirus. The new paper, published in the prestigious journal NPJ Vaccines, suggests that we can exploit these bacteria by using bits and pieces of them in vaccines.

Vaccines work by stimulating an immune response, specifically the part of the immune system responsible for producing antibodies. Small molecules called adjuvants are typically added to vaccines to make them more potent and, hence, elicit a stronger immune response.

Unfortunately, the "gold standard" of adjuvants - an aluminum salt called alum - is no longer seen as adequate because it does not activate all of the necessary components of the immune system to elicit a robust mucosal antibody response. Specifically, it does not activate Th17 cells, which play a central role in the production of antibodies in mucosal surfaces, such as those found in our mouths and noses. (Protecting these surfaces is key to stopping infectious respiratory diseases like influenza and COVID.)

Worse, there has been little progress over the last few decades in the development of new adjuvants, largely a result of the steep regulatory hurdles required for approval.

Until new adjuvants are developed, vaccine technology will be severely limited.

That is why Gingivax, a new company centered around the benefits of the oral microbiome, is investigating oral bacteria as a potential source of new adjuvants. Molecules extracted from these bacteria, which can activate crucial Th17 cells, could be added to vaccines to increase their efficacy.

This isn't the first time that bacterial components have been investigated as potential adjuvants. Previous research examined the use of gut bacteria, but the adjuvants derived from them can cause serious adverse events in patients. Because safety is the top concern in vaccine development, an alternative path is needed.

That is why we have turned to oral bacteria. Research indicates that the adjuvants derived from them can activate the immune response without overstimulating it and triggering adverse events.

Gingivax is currently seeking grants to conduct research in the development of novel adjuvants.

Skin cancer is highly preventable. Over 90% of all skin cancer is caused by exposure to ultraviolet radiation from the sun or indoor tanning devices.

Americans can dramatically reduce their risk of skin cancer by:

* Not burning or tanning intentionally - no tan is a safe tan

* Generously applying sunscreen (remembering to reapply every two hours)

* Wearing sun-protective clothing

* Seeking shade during peak times of the day

* Using extra caution near water, snow and sand

The Pap Corps Donates $2 Million to Sylvester Comprehensive Cancer Center

DEERIELD BEACH, Fla. /ScoopCloud/ -- Overcoming the numerous obstacles imposed by the pandemic, The Pap Corps Champions for Cancer Research displayed their indomitable spirit during their May Presidents Zoom meeting when they announced a $2 million donation to Sylvester Comprehensive Cancer Center. The nonprofit has made numerous multimillion dollar donations to support research at South Florida's only NCI Designated cancer center; however, this year's gift required tremendous determination and innovative fundraising.

"I cannot tell you how grateful I am for the way our board and our 20,000 members rallied during these difficult times," said Susan Dinter, chair of The Pap Corps. "While COVID-19 stopped many things, it did not stop cancer or our members! I am so proud of the incredible resourcefulness of our wonderful volunteers and am thrilled that during National Cancer Research Month we were able to make this substantial donation."

Pap Corps members, throughout all 50 chapters, created virtual versions of their usual fundraisers and also developed new strategies, events and campaigns to raise money. In addition, the launch of the Giving Train monthly donation program, brick and bench sales at The Garden of Hope in Deerfield Beach and the growth of events in Palm Beach helped the organization develop new paths for fundraising.

During the Zoom meeting, Dr. Stephen Nimer, Director of Sylvester Comprehensive Cancer Center spoke to the members and thanked them for their commitment and generosity.

"While we were unable to gather in person for this check presentation, I know everyone felt a tremendous sense of accomplishment," stated Sally Berenzweig, who assumed the role of CEO in January 2021. "Working with these incredible volunteers during the past months has truly been an inspiration, and I look forward to what we will achieve during the months ahead."
